Your first appointment with our practice is a very important educational visit for both the patient and the practice. During this time we inform our patients regarding items of interest relative to current concerns. Discussion of the surgical and anesthetic procedures involved in your care are handled in a thorough manner, to give you a sense of ease and confidence during your treatment. Many procedures can be completed during the initial visit. For more extensive procedures, additional visits may be required.
IMPORTANT
For any patient under the age of 18 a parent or guardian is required during the initial consultation as well as on the day of treatment.
Please bring the following items with you on the day of your visit:
REFERRAL SLIP: That shows the reason for your visit.
MEDICATIONS: Information pertaining to the medications you are taking on a daily basis.
MEDICAL CONDITIONS: Information pertaining to any medical conditions that you are currently experiencing. If you are unsure of conditions that may be of concern during your visit, please notify our office during your consultation.
SURGICAL HISTORY: Information pertaining to any surgeries you may have had in the past.
INSURANCE: Include all medical and dental insurance information that applies to your account. Providing this will allow us to assist you in receiving reimbursement from your insurance company.
X-RAYS: If you have x-rays relevant to your treatment, please bring them with you.
